The Oregon Health Authority Transformation Center invites you to join coordinated care organizations, health leaders, policymakers, providers and community stakeholders for a two-day meeting to:

    • celebrate successes,
    • share lessons learned, and
    • inspire future innovation based on Oregon’s coordinated care model.

When: Wednesday, December 3, noon – Thursday, December 4, 5 p.m.

Where: Oregon Convention Center (777 NE Martin Luther King, Jr. Blvd., Portland, OR 97232)
