To save money on health care, Oregon Health Plan looks to trim hospital, emergency room visits

Writing in The Oregonian, Bill Graves uses the personal stories of Oregon Health Plan patients to shed light on health reforms being proposed in Oregon. CareOregon’s Community Care Team is a 5-year-old pilot program that provides outreach workers to help keep patients healthy and out of the hospital. It’s work foreshadows some of what is proposed for coordinated care organizations in Oregon’s SB 1580.
